User manual of the Medima Line administration sets

Privacy Policy

In order to meet the requirements of Regulation 2016/679 of the European Parliament and of the Council (EU) of 27th April, 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data (called GPDR), we kindly inform you that:

1. The controller and processor of your personal data is Medima Sp. z o.o. with headquarter in Warsaw: Aleje Jerozolimskie 200, 02-486 Warsaw, POLAND, entered in the register of entrepreneurs kept by the District Court for the Capital city of Warsaw under KRS No: 0000201189.

3. The purpose of processing your personal data:

a)Execution of contracts and commercial agreements regarding products and services offered by Medima Sp. z o.o. (legal basis - Article 6 (1) (b) of the GPDR).

b)Performing after-sales service for Medima Sp. z o.o. products (legal basis - Article 6 (1) (b) of the GPDR).

c)Marketing and promotion of products offered by Medima Sp. z o.o. (legal basis - Article 6 (1) (f) of the GPDR).

d)Handling complaints (received from customers and made by Medima Sp. o.o. against its contractors), as well as investigation and defense in the event of mutual claims (legal basis - Articles 6 (1) (b) and (f) of the GPDR).

e)Fulfilling the requirement to identify the supply chain, resulting from Art. 25 of the Regulation 2017/745 of the European Parliament and of the Council (EU) 2017/745 of 5th May, 2017 on medical devices (legal basis - Article 6 (1) (c) of the GPDR).

4. Providing personal data is mandatory for customers and contractors who are natural persons and persons representing the buyer or contractor in order to conclude an agreement with Medima Sp. z o.o.

In other cases (business contacts, service and warranty service, etc.), providing data is voluntary and aims to facilitate cooperation and fulfillment of mutual obligations.

According to the information on the Medima Sp. z o.o. website we assume that every person who gives his personal data while contacting us in commercial matters consents to their processing in the scope of services offered by the Company.

5. Your personal data will be processed for the period necessary for performance of the contract, and after this period for the purposes and for the time and to the extent required by law or to secure any claims, or until the consent is withdrawn.

6. The recipients of your personal data will be:

a)Employees and associates - to the extent necessary to execute contracts and commercial agreements as well as after-sales service for Medima Sp. z o.o. products.

b)Courier companies - to the extent necessary to deliver or collect a parcel containing products, parts or documents.

c)the President of the Polish Office for Registration of Medicinal Products, Medical Devices and Biocidal Products - within the scope defined in the Regulation 2017/745 of the European Parliament and of the Council (EU) of 5th April, 2017 on medical devices.

d)Public administration bodies, if it results from a legal provision, an administrative decision or a court order.

e)Entities providing and supervising IT systems of Medima Sp. z o.o. - to the extent necessary to service these systems.

7. Your personal data is not transferred abroad, we do not perform automatic data processing and profiling, we do not collect sensitive data or personal preferences and interests.

8. Every person to the extent resulting from the provisions of law, has the right to:

a)access to their data and their rectification,

b)removing or limiting the processing or opposition to processing,

c)data transfer,

d)withdrawal of the consent granted at any time (withdrawal of consent does not affect the legality of the processing, which was made on the basis of the consent prior to its withdrawal).

Considering the purposes of data processing by the Company and the requirements relating to medical devices, request for deletion of data, processing restrictions or objections to data processing may not be possible.

In accordance with Art. 19 of the Regulation, the processor may also refuse to delete data if it is impossible or requires a disproportionately large effort.

9. In case of any doubts related to the processing of personal data, please contact Medima Sp. z o.o. to receive relevant information.

Notwithstanding the foregoing, everyone has the right to lodge a complaint to the supervisory body - the President of the Office for Personal Data Protection.
